Name of Effect: Should be less than a sentence. I know some people like to name effects.
What does it do?: Go into detail of the full effect. Even if it is simple.
Restrictions(optional unless asked by staff): Its in the name. If the effect is a modifier effect it must be stated here.
Card Class: Red, Green or Blue. This is needed so us staff can put it somewhere.
